hereby state that all donations I have made to the DEMINED Trust since 6 April 2000, and all donations I make to the same hereafter are to be treated as Gift Aid Donations, and I want the DEMINED Trust to reclaim tax on such donations.

I am a UK taxpayer and understand that I must pay an amount of Income Tax or Capital Gains Tax equal to that reclaimed by the DEMINED Trust from my donations (currently 28p for every £1 given).

I will advise the DEMINED Trust of any change in the above information - and should I wish to cancel this declaration in the future.
